Release of Information
The Release of Information form authorizes KHS to send transcripts to the school (or schools) of your choice. Please note: This form will need to be signed by a parent/guardian unless the student is 18 or older. Submit the complete and signed form to Mrs. Holmquist to ensure transmittal of transcripts.
